| 中文摘要 | 精神科病人的嗆食事件嚴重危害病人生命安全,因此預防嗆食是精神科照護中不可忽視的議題。本院急性精神科病房2023年住院病人發生嗆食事件共有73件,嗆食率達20.6%。分析原因有:一、缺乏標準作業規範及管理機制;二、缺乏系統性教育訓練;三、護理師嗆食認知不足及評估技能完整性低;四、評估工具無法有效篩檢出嗆食高風險病人;五、缺乏合適衛教指導工具。本專案透過:一、制定完整的標準作業規範;二、建置查核表及監測機制;三、舉辦系統性教育訓練;四、修訂嗆食風險評估表及評估流程;五、運用海報及AI系統製作影音衛教影片等策略施行後,精神科住院病人的嗆食發生率從20.6%降至2.5%,達到專案的目的。本專案製作的影音衛教影片,也可提供作為後續精神科居家病人衛教素材之參考。 |
| 英文摘要 | Psychiatric patients’ choking incidents severely harm their safety. Therefore, preventing choking is an essential issue in psychiatric care. In our hospital’s acute psychiatric ward, there were 73 incidents of choking among hospitalized patients in 2023, resulting in a choking rate of 20.6%. The contributing factors were as follows: 1. lack of standardized operating procedures and management mechanisms; 2. lack of systematic education and training; 3. inadequate awareness of choking risks among nurses and insufficient assessment skills; 4. assessment tools that fail to effectively screen for high-risk choking patients; and 5. lack of appropriate educational guidance materials. This project addressed these issues by: 1. developing comprehensive standardized operating procedures; 2. implementing checklists and monitoring mechanisms; 3. conducting systematic educational training; 4. revising the choking risk assessment form and evaluation processes; and 5. utilizing posters and Using AI to produce health education videos. As a result, the incidence of choking among psychiatric inpatients decreased from 20.6% to 2.5%, achieving the goals of the project. The educational video produced in this project can also serve as a reference for future educational materials for psychiatric patients receiving home care. |
