醫院護理人員對我國長期照顧十年計畫認知之研究

The Awareness of Hospital Nurses on Taiwan’s 10-Year Long Term Care Plan

背景：臺灣於2007 年制定「長期照顧十年計畫」，開啟國家為長期照護需求者提供解決方案。而護理是長期照護品質的關鍵專業，護理人員對長期照護政策的認知將直接影響其對病人之照顧品質及資源利用，但是目前相關研究論述付之闕如。目的：瞭解護理人員對長期照顧十年計畫之認知情形。方法：本研究採橫斷式調查研究，以新竹地區某區域教學醫院之護理人員為對象，有效問卷為300份（回收率90.9%）。資料以SPSS 15.0 進行統計分析。結果：受測者平均年齡為31.97 歲，未婚者居多（52.0%），教育程度以科技大學畢業占多數（54.4%）。受測護理人員平均工作年資為7.35 年，對於長期照顧十年計畫政策的認知平均得分為48.54 分（總分100 分）。醫院護理人員對長期照顧十年計畫的認知在年齡、服務年資、婚姻狀況、專業能力進階層級、工作單位、在職進修狀況以及聘僱情形上有顯著差異性。結論：本研究結果顯示，護理人員對於長期照護的政策與資源認知仍有提升的必要。研究發現工作經驗越資深者對於長期照護的認知越高，而工作經驗越資深的護理人員通常擔任主管的機率較高，在臨床直接接觸病人並擔負長期照顧資源轉介或教育的護理人員則更有必要加強其對長期照顧十年計畫的認知，本研究建議未來此議題應納入各級與各科室護理人員持續性教育。

Background: Taiwan’s 10-year Long Term Care Plan was established in 2007 at the national level as a solution to serve the needs of those in need of long term care services. However, nursing as an important long-term care service profession, and the awareness of long-term care policy and services among nurses, will directly affect the quality and access of long-term care. Yet, current studies on nurses’knowledge on long-term care are scarce. Purpose: This study aimed to understand the awareness of the 10-Year Long Term Care Plan among nursing staff in Taiwan. Methods: This study was a cross-sectional survey research with 300 valid questionnaires completed (90.9%) by nurses in Hsin Chu from a regional teaching hospital. Data were analyzed by using SPSS 15.0. Results: The participants had a mean age of 31.97 years and most were unmarried (52.0%) with education at technical college level (54.4%). The average length of work experience was 7.35 years. The participants’awareness of long-term care policies in Taiwan was low, as indicated by the total average score of 48.54 out of 100 points. The differences associated with nurses’awareness of age, years of work experience, marital status, professional competence level, work unit, on-job training and employment situation were significant. Conclusion: As the findings showed that long-term care awareness among nurses was limited and needed to be improved. Nurses with more experienced tended to know more about long-term care. However, patients who need long-term care knowledge and resources tend to be cared for by junior nurses. Therefore, future efforts are suggested to bring long-term care policy and resources a top continued education topic so nurses’awareness of long-term care can be enhanced.
