降低長期臥床病人失禁性皮膚炎發生率

Reducing Incidences Rate of Incontinence Associated Dermatitis in the Long-term Bedridden Patients

本專案為降低長期臥床病人失禁性皮膚炎發生率，本單位自2013年1月4日至3月4日發生腹瀉且同時產生肛門、會陰周圍皮膚受損者佔31.1%，若失禁性皮膚炎持續惡化，會導致皮膚破損甚至潰瘍，進而發生感染。現況分析造成失禁性皮膚炎發生率高之主因為：一、失禁性皮膚炎發生率31.1%；二、護理人員對失禁性皮膚炎照護認知正確性74.7%；三、護理人員執行失禁性皮膚炎照護行為正確性63.7%。經專案擬定措施：一、製訂失禁性皮膚炎護理照護流程；二、舉辦失禁性皮膚炎教育訓練；三、製作「ABCDE紅臀輕鬆護」護理衛教單。專案執行後結果為一、失禁性皮膚炎發生率下降至11.1%；二、護理人員失禁性皮膚炎認知正確性提升至95.3%；三、護理人員執行失禁性皮膚照護正確性提升至95.6%。

The thrust of this project is to reduce the long-term bedridden patients the incidence of incontinence dermatitis. Patients suffered from diarrhea induced anus, perineum surrounding skin damage incidence were 31.1% from January 4 to March 4, 2013 if sustained incontinence dermatitis worse, can cause skin damage and even ulcers, and thus infection. Analysis of the current situation caused by the main incontinence incidence of dermatitis because: (1) the incidence of incontinence dermatitis 31.1%; (2) nurses care for incontinence dermatitis cognitive correctness 74.7%; (3) the implementation of incontinence dermatitis nursing care behavior correctness of 63.7%. Methods: (1) the development of incontinence dermatitis nursing care processes; (2) hold incontinence dermatitis education and training; (3) the production of ＂ABCDE red buttocks easy care＂ nursing health education alone. Results: (1) The incidence of incontinence dermatitis after project execution fell to 11.1%; (2) incontinence dermatitis cognition correctness of nurses increased to 95.3%; (3) the implementation of incontinence skin care correctness of nurses increased to 95.6%.
